Welcome to the Team!

In order to be certified as a team for league play, the following paperwork must be collected from every player.

The following items will be signed online during registration and will NOT be handed in physically:

Rosemont waiver
PRHF waiver
Media waiver
PRHF Parent/Player Contract

The following items must printed out and physically handed in on equipment pick up day:

Player birth certificate – can be a certified copy or a clean legible photocopy
Physical signed by a medical doctor with a 2026 date
